Background: Benign pneumoperitoneum is a common finding after endoscopic procedures, associated with rates as high as 25% after PEG placement. However, tension pneumoperitoneum following PEG placement has not been reported in the literature. 
Case presentation: Here we describe the case of a 31-year old female with history significant for suspected myotonic dystrophy and tracheostomy placement who developed tension pneumoperitoneum shortly after PEG tube placement. She required immediate diagnostic laparoscopy during which no perforation or solid organ injury was discovered. After decompression of the intra-abdominal free air, she recovered well within 24 hours with no further sequella.
Conclusions: Tension pneumoperitoneum is a possible complication of PEG placement.
